In 2017 two employees were cutting pipes with an angle grinder on top of an eleven-metre-high slurry tank without fall protection; sparks ignited gases, the tank exploded, both were seriously injured and one had a leg amputated. There was no safe tank design, no risk assessments, no permit-to-work procedures, no training and no supervision, and the operator also breached its environmental permit; fines totalling 304,500 GBP plus 229,988 GBP in costs. The company is a micro-enterprise with fewer than 10 employees.
Carry out hot work on plant containing flammable gases only with a permit to work, gas testing and fall protection – having a permit on paper does not protect against liability.
