While a high-pressure jetting system was being primed in freezing conditions in 2022, a pressure release occurred and the end of the hose struck a drainage engineer in the face; he died two and a half weeks later. There was no safe system of work, jetting vices were not properly maintained, monitoring was lacking, and information, instruction, training and supervision were insufficient; fine of 800,000 GBP (reduced from 1.2 million GBP for an early guilty plea) plus 8,680 GBP in costs. According to its consolidated accounts (financial year to July 2024), the company employed 2,268 people and the group 3,610.
