In 2021 a 45-year-old employee in Penistone became entangled in an unguarded wire drawing and recoiling machine and died. There was no risk assessment, no safe system of work, no fixed guards or interlocks and no formal training, only verbal instruction; after the accident eight prohibition notices were served. Fine of 140,000 GBP plus 6,652 GBP in costs for the business, which had 17 employees according to its annual accounts.
Provide formal training and physical guarding rather than relying on verbal instruction – make dangerous moving parts technically inaccessible.
