At the Alloa glassworks, a shovel loader used to clear molten glass waste and hot water had been missing the protective cab door for almost two years, although the damage had been reported and other drivers had already been struck. In 2024 material spilled into the cab and scalded a 32-year-old; fine of 600,000 GBP. According to the HSE (Health and Safety Executive, Britain's workplace safety regulator), around 500 people work at the site; according to its 2024 annual accounts, the company employed an average of 642 people.
Set up a binding repair process for reported damage to safety equipment – a report that is not acted upon is a documented failure.
