12 Dec 2025 EllisDon CorporationEllisDon: CAD 514,831.53 penalty after fatal crane incident at high-rise site Canada, British ColumbiaWorkplace safety and accidents €318,998

In February 2024 a flytable was being moved by tower crane at the Oakridge Park construction site; as it was pushed out it accelerated out of the building, fell and fatally struck a worker. WorkSafeBC found that EllisDon, as prime contractor, had not regularly reviewed the subcontractor's safe work procedures, had not confirmed that a risk assessment had been carried out, had not classed the lift as critical, had not ensured written lift plans, had not controlled the exclusion zone on the ground and had not coordinated the health and safety activities of the employers involved. It imposed an administrative penalty of CAD 514,831.53.

What organisations can take from it

Prime contractors must actively plan, review and coordinate their subcontractors' critical crane lifts rather than relying on their procedures.
