In February 2024 the formwork contractor was moving a flytable by tower crane at the Oakridge Park construction site when it fell out of the building and fatally struck a worker. WorkSafeBC found, among other things, missing written lift plans for critical lifts, inadequate procedures for corner flytables, a lack of risk assessments and inspections, loads being passed over people and insufficient instruction, training and supervision of workers, and imposed an administrative penalty of CAD 112,624.66.
Formwork and lifting crews need written step-by-step procedures, trained ground control workers and controlled exclusion zones.
